UNA LA MIA STRUTTURA METAFISICA Livelli elevati di strutture anomale scoperte nel sangue dei pazienti con Covid Lungo Tra le conseguenze non ancora pienamente comprese dell’infezione da coronavirus SAR-CoV-2 vi è il Long Covid (Covid Lungo), un insieme di sintomi – come la famigerata nebbia mentale e l’affaticamento cronico – che può durare mesi o addirittura anni e che colpisce una parte dei pazienti esposti al patogeno responsabile della pandemia di COVID-19, in particolar modo quelli che hanno sviluppato la malattia grave. Un nuovo studio ha scoperto un potenziale biomarcatore di questa misteriosa condizione, strane strutture nel sangue che in test di laboratorio sono state utili a identificare le persone affette da Long Covid con una precisione superiore al 90 percento. In parole semplici, queste strutture sono corpuscoli derivati dall’unione di microscopici coaguli di sangue (trombi) e trappole extracellulari dei neutrofili o NET (acronimo di Neutrophil Extracellular Traps). Sono strutture fibrose e filamentose composte da cromatina non condensata (DNA con istoni) e proteine ad azione antimicrobica come Mieloperossidasi (MPO), Proteinasi 3 ed Elastasi neutrofila. Vengono rilasciate dai neutrofili – un tipo di globuli bianchi o leucociti – per intrappolare come una rete patogeni alla stregua di batteri, parassiti, virus e funghi, un po’ come una ragnatela di Spiderman. In genere le NET si degradano rapidamente dopo aver svolto il proprio prezioso compito di proteggere l’organismo, ma nei pazienti affetti dal Long Covid diventano persistenti e in concentrazioni molto elevate. A determinare che nel sangue dei pazienti affetti da Covid Lungo si trova un’elevata concentrazione di anomale strutture derivate dalla fusione di coaguli di sangue e NET è stato un team di ricerca internazionale guidato da scienziati dell’IRCM – Istituto di ricerca sul cancro dell’Università di Montpellier (Francia) e del Dipartimento di Scienze Fisiologiche – Facoltà di Scienze dell’Università di Stellenbosch (Sudafrica). I ricercatori, coordinati da Alain R. Thierry e Etheresia Pretorius, sono giunti alle loro conclusioni dopo aver approfondito i risultati di precedenti indagini. Come sottolineato dagli autori nell’abstract dello studio, infatti “la persistenza di complicanze vasculo-trombotiche è stata proposta come possibile fattore contribuente alla sindrome da COVID-19 Lungo”, alla luce di ciò che emerso dalle analisi del sangue. La presenza di microscopici coaguli in questi pazienti era stata già osservata nel 2021, mentre l’anno successivo sono stati osservati i livelli elevati di NET. La nuova ricerca ha coinvolto i campioni di sangue di decine di persone con Long Covid e sane, che sono stati sottoposti a nuove e approfondite analisi di laboratorio come la citometria a flusso per immagini e la microscopia a fluorescenza al fine di far emergere le differenze. Ebbene, gli studiosi non solo si sono accorti che le concentrazioni di microtrombi e NET erano sensibilmente più elevate nei pazienti Covid – per i primi di circa venti volte superiori – ma che queste strutture si “fondevano” dando vita a un corpuscolo anomalo, con le trappole extracellulari dei neutrofili incorporate all’interno dei coaguli di sangue. Tali strutture in realtà sono state rilevate anche nel sangue delle persone sane, tuttavia in concentrazioni molto ridotte. Non a caso, utilizzandole come un biomarcatore e sottoponendole a un’intelligenza artificiale, le strutture fuse di microcoaguli e NET sono state in grado di determinare la presenza di Long Covid con una precisione del 91 percento. “Questa interazione potrebbe rendere i microcoaguli più resistenti alla fibrinolisi (il meccanismo biologico che degrada naturalmente le NET NDR) favorendone la persistenza in circolazione e contribuendo a complicanze microvascolari croniche”, ha affermato in un comunicato stampa la professoressa Pretorius. “Questa scoperta suggerisce l’esistenza di interazioni fisiologiche sottostanti tra microcoaguli e NET che, se disregolate, possono diventare patogene”, le ha fatto eco il dottor Thierry. I trombi nei pazienti Covid risultavano anche più grandi. Chiaramente serviranno studi più approfonditi per capire come questi corpuscoli persistenti possano innescare i devastanti sintomi del Long Covid, tuttavia aver fatto emergere il potenziale meccanismo sottostante potrebbe migliorare la diagnosi ma soprattutto far trovare una possibile cura, magari attraverso farmaci in grado di dissolvere le strutture anomale. “Questi marcatori hanno mostrato un’ottima performance diagnostica, sia singolarmente che in combinazione Il nostro studio ha rivelato che i NET potrebbero essere una componente dei microcoaguli circolanti.
